    It is about a man who is now laying in his death bed remembering what an awful sinner he was in his youth so now he wants someone to tell him hes a good man so he can (think that he will) go to heaven. I'm 14 and i figured this out... but He realizes moments before his death that what he did was unforgivable and that he is doomed to hell.

    its simple. he has done something wrong, and now is repentant. he thinks he is going to hell, and needs reassurance.

    I think it's about someone-- and since this is on The Black Parade album, I'm guessing it's The Patient-- who sinned at some point, and now that he know's he's going to die, he wants to be forgiven. And he's not sure if he will be, that's why it keeps going from "tell me I'm an angel" to "tell me I'm a bad man". But then near the end the angel part goes away and he's just saying he's a bad man, so he figures that he wont be forgiven and that he is doomed to hell.

    he's gonna die and he's afraid of going to hell so he wants some one to tell him that he deserves to go to heaven

    Its was based on that episode in loony toons when the cyote goes down to the underworld and he findes himself in a jazz club where they are all wolves
