This guy has a girlfriend going to bed with him, however she possibly has something affecting her personality (drugs? alcohol?) and he's just using her for the night. He'll wake up to somebody sober, a whole new person than the one he went to bed with, and her sober eyes are much more beautiful than her other persona.

Again referring to the fact that they both contributed to the downfall and how sad it makes her that it couldn't be saved.

She's been depressed possibly, her life moving slower since they started going downhill. She could break their pattern by calling him and ending everything, they could easily find a reason to end things but she knows that no matter the reason, he will always have a part of her with him.

She's saying they both had a part in their failing relationship and she's regretful of that fact. It feels like her heart has been taken away. "A looser grip" refers to their holding hands, gradually their grip gets looser and looser, less intimate, but neither of them notice and eventually she ends up pulling away completely when they go for each others hand.

Her ex is telling her she won't have time for him, trying to convince her to keep to herself, possibly showing he doesn't want to be in contact with her, but she's insisting that time is all she has now that he's gone.

The needle dragging thread is a metaphor for their relationship unwinding. Unraveling the chapters of their life and they can't receive help because only they know how those chapters were formed. But the threads will unravel out of control and cast a shadow over the small pieces of hope that are left for them.

I can't say for sure what the 4 to 1 means, but I take it as a general metaphor for her losing things important to her over a long period and she can't afford to lose anything else close to her. She may feel like she's about to lose her significant other and she wants to keep them company until the moment they fall asleep because they're the only thing she has left.
