Walking through life one encounters many hardships and bumps in the road, and this verse depicts how the singer has made it through many of these over the course of their life; "Through many dangers... I have already come". The second part of the verse links it back to God, describing how it is His Grace that has brought the singer through all these hard times. Grace is what saves each and every one of us; the sheer brilliance of God is what gives someone the strength to make it through adversity. "Grace will lead me home" means that the Grace will bring the singer to the end of their life, and into the kingdom of heaven/"home". There they will be with God again, and this is the ultimate definition of Home.
