It is about his rehab and getting of the drugs. I have to do a song analysis on this song for school :)

Most of the album (except for "in one ear" and perhaps "judas") is about change, relations between the past and present in particular. This song in about fear of the past catching up with you, trying to forget it- "burn all the evidence of fabricated disbelief", "want to get away, i need to get away". He wants to burn down his past, start over, get as far away from his past as he possibly could- "i'm stuck in right between the shadows of my yesterday".
He fails in the end- hence the title and the chorus. Think about it.

The song could apply to a number of scenarios i don't believe it is meant to apply to just one scenario but have a general idea that many people can relate to. The song revolves around the idea of feeling trapped and others are manipulating you. Some main scenarios this could apply to include a girlfriend, a job, an addiction, or just life. I think the song is either directed towards a specific person or god asking why they were put in this situation.
