In the part where he says "alright, and if you still won't go tell your sister that I told her so" I think it's safe to say the girl's sister knew, and that the sister and the writer had spoken about the user going to rehab, though the writer predicts she won't try. "Tie the shirt into another bow; Take a dip into the stash you stole" tying the shirt into a bow around a leg or an arm would create a tourniquet, which is consistent with heroin use. And I suppose we can assume she couldn't supply herself, so she stole the drugs or enough money to purchase them. In the beginning, the writer says "So I woke up with a curse; I was searching, I was looking through the purse. Taking measures to defend, Because you swore you'd always treat me like a friend." He had his suspicions, and maybe even dreamed about it. But upon looking through the user's purse, his suspicions were confirmed. He either hid or disposed of the heroin to stop her from using it, taking measures to defend her from herself, because he feels she'd do the same for him.

    He is trying to ride out a detox with her, but there is more to their relationship. He is a wishy-washy co-dependent who has placed her on a pedestal and has been stuck in the "friend zone" for years. Her family tried to explain her drug addiction to him, but she is able to walk all over him because he thinks that she could do no wrong.

    There’s a line in an article about them that states ““You Might Be Right” is a sneaky ode to defeat and accepting that you don’t have a chance with a girl and giving up”” and I’m inclined to believe it. If this is true, I assume part of the lyrics are him wanting to not give up despite being rejected (golden girl line) part of them are angry at her for not giving him what he feels he believes (accusations of stolen stash) and part are him accepting (you might be right).
