This is what I think,
He feels like a boy in a box, because he takes meds for his ADD or ADHD. The meds make him feel like his body isn't controlled by him but he just watches from the outside, like sitting in a box.
You have to watch the time, cause when the meds don't work anymore you have to take another pill, drink it and swallow.
