I had trouble highlighting the line I wanted so role-playing it here because it was bugging me and the answer wasn't on Bing so hopefully the next person finds this helpful. Wish in one hand sh*t in the other is the line I wanted to explain. The speaker is doing meth or methamphetamine and sees his life at a turning point where he could go on to be successful and fulfill his dreams if he stayed away from the meth or on the other hand could find himself in a life of struggle and hardship due to his addiction to meth. He seems indifferent to the outcome.

She has a job Monday through Friday. Her weekends are precious and she has to make the best of the short weekend. She travels and enjoys the weekends but the stress of showing up and being a dependable person for her 40hr lifestyle is taking its toll. It makes her irritable. It would be hard to go to Germany and back to see her soccer friends and make it back in time to clock in Monday morning but that's how she's trying to live.
