Adele herself has explained that the song is from her older self to her younger self. The lyric "both of us are running out of time" is a warning that fame, for all its costs (which she's apologizing for, though recognizing that it's an ongoing choice: "it clearly doesn't tear you apart") is still a fleeting thing.
