This song to me describes the living hell of suffering with mental illness, you lose control of yourself and do things you never thought you would be capable of doing - to yourself and other people. This song is like someone has gone through a suicide attempt ( "I cant escape this hell, so many times I've tried") and has lost grip on their self control and feel like a monster, an animal. Just like in the video where he smashes up the bar, some people get psychosis and lose grip completely and see monsters in themselves and the people around them.

This song is almost the voice of addiction, the voice that urges you to take that pill or drink that bottle. It describes how someone is so addicted to something they cannot escape it, even though it could kill them and be the last thing they do. It describes how someone's emotional agony forces them into trying to numb their pain/life out with an addiction. From a personal perspective of dealing with a Codeine (painkiller) addiction, this song describes exactly how the addiction feels "The shoulder you cry on" and is pretty much the way my addiction speaks to me.

To me this song describes a suicide attempt (The needles breaking your skin, the scar is sinking in) being saved in a hospital, and coping with mental illness in which you feel you are on the edge of life, close to losing yourself completely. "It's All Over" describes (to my ears) a suicide attempt that was unsuccessful but has left physical and emotional scars behind, not just from the person being described in the songs perspective but that of the singer too. "And it gets under my skin, to see you giving in" - could be a reference to a person seeing someone they care about falling prey to the twisted ideas and impulses that mental illnesses can put in someone's head, making them do terrible things to themselves and inadvertently hurting those around them.

and falling off
It's all over for you
For you!
When you're on
This could be a way of describing the way it feels to have mental illness, to be "on the edge" of sanity and having to balance the pain inside with the life outside. This certainly could speak for people who feel their life is over because they are mentally ill, especially those with Axis 2 diagnosis.

(Axis 2 Mental illnesses take years of medication and therapy but can be present throughout life ie Never Cured, examples of this include Schizophrenia and Borderline PD - opposite to Axis 1 disorders such as Depression and Anxiety, which while just as distressing, can be "cured" with medication and therapy.)

skin
To see you giving it
And now your trip begins
The lyric is actually "And it gets under my skin, to see you giving in" - This again could be a reference to the perspective of a person seeing their loved on going through a suicide attempt, they are giving in to their illness/impulse and hurts the person who has this perspective.

Explanation
"
this can't go on
Because of you my mind is always racing
The needles breaking your skin
The story sinking in
And now
This could be from the perspective of someone watching a loved one attempting suicide. "The Needles breaking your skin" could be a reference to the needles being inserted in a hospital after an overdose (canulars, a drip feed). The lyric is actually wrong, it is "The Scar is sinking in" NOT "the story sinking in" - which could be a reference to how attempting suicide changes a person and leaves both physical and mental scars behind.
