I thought it was more simple than that. I see it as he is totally in love with her, but she's not totally in love with him like she previously thought and she's realizing it when this other guy is calling her name. He's jealous of this other guy, and his out of control lifestyle is tearing them apart even more. Thus he's the one of the two that feels like he's in love, everybody knows this yet they're together. It's no secret that she doesn't love him.
I think it's about confusing dependency for love. His gotten emotionally stuck and addicted to holding on to her with the only way he knows how to, through playing the old "I'm a disaster case come save me". I think subconsciously he does this because he needs to be rescued by her inorder to feel loved. That the only time, he really feels loved by her is when she is saving him. And because he is so disconnected from himself he doesn't connect that, his external circumstances are a reflection of his internal world. It is his fear of losing her, that is causing his world to spin out of control. Everyone else can see this but him.

Two people who came together when they were at their darkest, fell in love. But the other doesn't feel they are capable of love or deserving. But everybody knows, they are actually both perfect for each other and are truly in love with each other if they would just give into it. They spin out of control because they are so meant to be together but it is confusing because nothing else makes sense but when they are together. And everybody else sees this but them.
I think it means the person is in a bad, bad place, but they sill love someone. It's haed to get the person you love to be in that place with you because it's ugly...no one wants to be there, and it gets lonely, and you feel even more out of control.
