    it could possibly be about drugs "the needle tears a hole" and shooting up makes him numb so he hurts himself just to feel something. or maybe he just feels like he's losing himself and his friends. "i will let you down, i will make you hurt" could mean someone has caused him so much pain and to believe he's a bad person and he wants them to know his pain.
