If you watch the video its easy to figure out the meaning. Katy's boyfriend is in the army and goes to fight. He is killed in action. Then Katy is with someone else, but she keeps wishing he was her first boyfriend. Her first was perfect, and now she says 2nd best is all she will ever get because the very best died. She regrets letting him leave.
