As one can clearly judge from Nirvana's live performances, Kurt was not a guitar virtuoso (does not necessarily mean he couldn't play the instrument well when sober).
For Teen Spirit solo, Kurt basically copied the vocal melody of the verse and pre-chorus, using Electro Harmonix Small Clone Chorus guitar effect pedal to achieve the signature watery sound (also prominent on such Nevermind tracks as "Come as You Are" and "Drain You").

Some sources indicate that Kurt sings "miskito" instead of "mosquito" with Miskito being an indigenious tribe in Central America, however there's no factual proof of that, since Nevermind album booklet did not contain full song lyrics, only collage of those.

On numerous occasions Kurt admitted that "music comes first and lyrics second". He would even describe his own lyrics as "a big pile of contradictions. They're split down the middle between very sincere opinions that I have and sarcastic opinions and feelings that I have and sarcastic and hopeful, humorous rebuttals toward cliché bohemian ideals that have been exhausted for years."

This line is about Kurt's then-girlfriend Tobi Vail, member of riot grrl band Bikini Kill.
Many of the songs on "Nevermind" touch Kurt's relationship with Tobi. In fact, it was Vail's bandmate Kathleen Hanna who spray painted "Kurt Smells Like Teen Spirit" on Cobain's wall, thus coining the song's name. Kurt liked the slogan and thought it had revolutionary vibe. But what Hanna really meant was that Cobain smelled like the deodorant "Teen Spirit", which Tobi Vail wore.

Contrary to the popular perception of the song's meaning as a teenage anthem, it was actually mocking the then-current Generation X, describing the early 90s youth as bored losers, who are somehow proud of having nothing else to do rather than slack around and shoot guns, although the problem of school violence won't reach the spotlight until Columbine shooting in 1999.
