The reason it asks "who's in control now" is because the anorexia is taking over her life. This is very common with eating disorders, where they start taking over you're life, whether you want them to or not. That is one of the reasons people who overcame an eating disorder are likely to relapse and go back to their eating disorder.

The secret kept within her is the eating disorder she is struggling with. The eating disorder sounds like anorexia nervosa, and the reason she's hiding it is because she knonts out, they will try to get her to stop starving herself, which she feels she needs to do in order to be perfect. What she doesn't know is she's already perfect, because everybody says she's a winner.
