I think this line is referring to cuts and scars that he has after the abuse he receives from his lover. His mother is worried about him and his relationship but he reassured her that everything is fine. "It looks ugly", the cuts are gross and could be gory. "But it's clean", he could be a little defensive about it. Like there's nothing to worry about. It's a little scratch, I could clean it up.
