I think that the meaning to this is that he dreamt this. "i decided to name her insomnia." Insomnia is where you don't get a normal amount of sleep. Also dreams have different and weird things in them, "I didn't think the clock struck more then twelve times." It also describes what she looked like. "Her teeth like white seeds in a scarlet fruit.", and "This i must tell you, old friend: fear beauty." So to me its a dream about a girl with very white teeth and who wasn't the prettiest thing ethier.
