He can almost smell the decay of the fruit. Which to me means the forbidden fruit..lust/love on the side. And he uses the word decay as it has been a while since they last shared the forbidden fruit. She takes care of her husband or boyfriend as she is supposed to within her role as the significant other. And just as a spouse or wife. She undresses and shares a bed with him. I believe the writer is saying that he knows they are together at night and that possibly they are intimate. But he wonders if she shares the same secret desires with her mate as she does with him.
