The man had a secret to tell a loved one. The "secret" occurred 20 years ago (my guess is it was an affair from the sound of it). Maybe the picture was of the mistress or even a picture of an illegitimate child. He wrote this letter those years ago telling the loved one of his actions. He didn't go through with giving the letter then for fear of losing everything. The man didn't know how to keep the secret any longer thus slipping the note under their front door. He loves his spouse dearly and would wait for 20+ more years for her response to his letter (instead of a dead silence with rejection) and he prays redemption for himself and their marriage.

I don't think anyones dead. It looks to me that the person writing the note disappeared/left, twenty years ago they wrote a note (i guess explaining why they left) and left it infront of the door (how it stayed there for twenty years? I don't know.) they know there's not a good reason for why "they left" but they'll wait, even if it takes twenty more years, hoping the recipient will write back and send a note to their front door and also praying for a day to redeem themselves/make it up to that person for leaving. Again, that's just my imagination.
