    James likes to write his lyrics in a somewhat crptic manner so more people can relate to them. there's a literal sense to them but they're actually about being trapped in whatever vices you have in your life and wanting to escape them but you can't seem to shake them. for some people it can be some kind of addiction they're trying to kick for others it might be somebody they want out of their lives or whatever. i think all the postings i've seen are right in a sense but ultimately they don't have a specific meaning. he also likes to write lyrics based on movies (One) or stories from the Bible (Creeping Death), or even childhood fears like nightmares (Enter Sandman) but there are many other souces he draws from. i heard him talking about it on XM radio right before Death Magnetic was released. for the most part his lyrics can be interpreted by anybody to mean something to them personally which explains his reference to Bible stories or movies and what-not. do some research and you'll find that James Hetfield really seems to get the human mind and how to relate to the masses in his lyrics.

    this song is about people in mental hospital's who feel that they are in a prison, they want out, the people who run the mental hospital's think there doing what's best when sometimes there just making people worse in there, making them do worse and want to be worse, and they want out, want some freedom.

    Personally, I feel that this song is about being insane; The 'sanitarium' being his mind. He says "build my fear of whats out there." meaning the sane world around him. 'They' may refer to the voices in his mind or people persecuting him. And last, 'Kill...seems the only way of reaching out again' he thinks murder may be the only way to stop the voices

    ("Violent use brings violence plans") violence only gets more violent, also that some people lose their minds cause they try to get back at those who used violence to do something to them. Basically violence twists the mind

    A man in an insane asylum wants to break out, and he tries to rally the other patients, but the doctors keep them in check, and this makes the patient angrier. The doctors think that their work is helping he patients, when it's making them want to break out more.
