Fun trivia fact, this song was actually inspired by john marsden's "one flew over the cuckoos nest" where the characters are trapped in a torturous mental institution ruled by a dominating and immasculating woman named "nurse ratced".

James likes to write his lyrics in a somewhat crptic manner so more people can relate to them. There's a literal sense to them but they're actually about being trapped in whatever vices you have in your life and wanting to escape them but you can't seem to shake them. For some people it can be some kind of addiction they're trying to kick for others it might be somebody they want out of their lives or whatever. I think all the postings i've seen are right in a sense but ultimately they don't have a specific meaning. He also likes to write lyrics based on movies (one) or stories from the bible (creeping death), or even childhood fears like nightmares (enter sandman) but there are many other souces he draws from. I heard him talking about it on xm radio right before death magnetic was released. For the most part his lyrics can be interpreted by anybody to mean something to them personally which explains his reference to bible stories or movies and what-not. Do some research and you'll find that james hetfield really seems to get the human mind and how to relate to the masses in his lyrics.

Personally, i feel that this song is about being insane; the 'sanitarium' being his mind. He says "build my fear of whats out there. " meaning the sane world around him. 'they' may refer to the voices in his mind or people persecuting him. And last, 'kill... Seems the only way of reaching out again' he thinks murder may be the only way to stop the voices

also to add on, when it starts to get near the end of the song if you read carefully. It starts to become about how his rage is building and he is feeling the need to kill people.

You need to correct this it's actually 'welcome to where time stand still' and the song is all about a mental home (sanitarium) and james is a mental person in first person view.

That's a very beautiful song and very truthful but in the lyrics you have written something wrong, it's not "welcome to where the world stand still" but " welcome to where time stand still" because if you listen attentively you will hear "time" and not "world" and try to sing it also, you will hear that it sounds better whit time ^^

This is based on "one flew over the cuckoo's nest," which, i'm assuming, features jack nicholson trying to escape an asylum.
