    add to my above comment: spoons may have a double meaning, because anything in that place can become a weapon very fast.

    bars on all the windows = mental hospital / rehab
    counting up the spoons = spoons are used to cook heroin
    hits me with a needle = sedative, easy way to deal with manic patients

    James Taylor himself talks about how this song was about one (his first?) trip to the hospital due to mental illness and drug addiction.

    Keeper's trying to cool me, says I'm gonna be alright (withdrawal)
    Feel him getting edgy every time I make a sudden move
    (how would YOU deal with a withdrawing heroin user you're talking down.)

    Friends come, can't understand why James is in a mental hospital, because "he's just like the rest of us" he's not crazy! But Remember, James checked himself in.

    I think this is a better, healthier, closer-to-the truth message than above.
