This song isn't necessarily about heroin specifically, "now i'm a slave to the black drug" does lend itself to that i suppose-- but nevertheless i think they mean downers in general whether it be opiates, weed, or alcohol. Ultimately it's definitely drug related... Everyone's got their own demon.

Pretty straight forward meaning. This song is about heroin/opiates, specifically withdrawl. "i thought myself a master of the arts. This dimension of misery is my penance". Seems pretty clear to me.
